I lost/can’t find my survey passcode.
You can find your passcode in the online survey access instructions included in the survey information email and/or letter we sent you. If you cannot find your instructions, please click here to fill out the survey support form and a member of our staff will send you an email with your passcode. Be sure to include your full name and your email address so that we can send the correct passcode.


What is a survey passcode?
Your passcode is an 11-digit unique code that allows you to access our online survey. Only individuals who have been invited to participate receive survey passcodes.


Error message "Survey no longer active."
This error message typically comes up when the Internet survey page has timed out due to being idle for a period of time. To resume your survey, close your web browser (e.g., Firefox, Chrome, IE), re-open the web browser, and go back to www.kantarhealthstudy.com. When you log back in the survey will continue where you left off. You may need to wait 10 or 15 minutes before your survey becomes available again.

Other error message
Below are some helpful hints for avoiding error messages or determining why you're getting an error message:

  • Never use the back button on your web browser. Only use the buttons provided in the survey.
  • If you receive an error message, read the question carefully and be sure to follow the instructions provided for that question. Certain questions require a response on every line or for every answer list item and the survey will not advance until all questions have been answered.

My computer crashed before I was finished. Do I have to start over?
Whether you leave the survey by accident or on purpose, you should always be able to pick up where you left off by going back to the survey site www.kantarhealthstudy.com and logging back into the survey. You may need to wait 10 or 15 minutes before your survey becomes available again.

Where can I find the link to the survey?
The link to the survey is on the Home page of this Web site. Click here to begin. To start the survey, please type your survey passcode in the box and click the "Go" button. Enter the survey passcode exactly as it was communicated in your invitation letter or email. If you experience difficulties starting the survey for the first time please fill out the Survey Support form and a member of our staff will contact you to help resolve the problem.


How much longer is the survey?
We appreciate your patience. Survey length can depend on your responses, but most people complete this survey in about 25 minutes. If you need to take a break, you can temporarily suspend the survey at any time by clicking the "stop" button at the bottom of any survey page. To resume your survey, go to www.kantarhealthstudy.com and log back in using your survey passcode. The survey will continue where you left off.


Is the Promise incentive legitimate?
As a thank-you for completing our survey, we will mail you the promise incentive mentioned in your survey invitation letter or email. Because it takes time to process our survey results, please allow 6-8 weeks for delivery. We appreciate your patience and participation!

What is the Health & Wellness Survey?
The Health & Wellness Survey is designed to understand how you use, and how much you value, different types of information sources (e.g. magazines, television, radio, the internet) when making health and wellness related decisions.


Is this a telemarketing or sales effort?
Absolutely not. This study is conducted for marketing research purposes only and we will never attempt to sell you anything.


Why did you pick me to participate in this study?
A few weeks ago, a member of your household filled out a paper survey for CRI that had colorful pictures of magazine covers on the first few pages and asked about media, lifestyle, and health and wellness. This is a follow-up study specifically for the same adult in your household who filled out the paper survey.

Please note that in order to receive the cash promise incentive mentioned in the invitation, this online survey MUST be completed by the same adult who filled out the paper survey. If you have any questions or concerns, please fill out the Survey Support form and a member of our staff will contact you to help resolve the problem.

All personal information provided to CRI/Kantar Media is combined and reported anonymously with information gathered from other survey participants.
